Congregate Housing Facilities are located primarily in Manhattan and The Bronx, with residences provided for single clients and for families.

• Cedar Tremont
  This uniquely designed apartment house opened in 1995, and includes supportive services for 17 formerly homeless families in which the head of household is diagnosed with a disability. Today this facility serves families with disabilities as well as homeless families referred from the Department of Homeless Services. Cedar Tremont families receive supportive services including case management, entitlement advocacy, health education, substance abuse counseling, educational and recreational services for children, adolescents, and adults. • Chelsea Court
  Located in Chelsea and opened in March 2003, this 18-unit permanent housing project provides studio apartments for 14 previously homeless individuals with disabilities and 4 community apartments available to persons with low incomes. • Dreitzer House
  Located in East Harlem and opened in June 1999, this supported living facility with one- to three-bedroom apartments is a permanent home for 36 formerly homeless families with a disability, such as substance abuse or mental illness. Families receive on site case management. The building also houses the Marshall Goldberg Learning Center. Residents have access to the library andcomputers, where adults can work on vocational skills and their children can do schoolwork. Tel: 917-492-1160


• Flora Vista
  Opened in early 2005, this East Harlem residence is a 20 unit apartment permanent supportive housing project. It provides on-sitecase management services to formerly homeless individuals with disabilities such as substance abuse and mental illness. Tel: 212-427-7881


• Fox Point
  Located in the Hunt’s Point sectionof the Bronx, Fox Point is one of the first residential buildings in the Bronx to get Gold LEED (Leadership in Energy & Environmental Design) certification for environmental sustainability. The on-site staff provide clinical case management, family-centered activities and employmentand vocational services, to 31 formerly homeless families disabled by substanceabuse, and 16 low income families and individuals from the community. • Hill House
  Opened in the fall of 1999, this building is designed for individuals who have personal histories of homelessness and a disability such as addiction. Hill House is located in the Bronx on a site adjacent to our Starhill facility. Tenants in the 43 units have access to support services that assist them with their health and nutrition, substance abuse counseling and other areas of need. Tel: 718-583-9824


• Jerome Court
  Opened in 2000, the 40 unit apartment building offers furnished studio apartments for previously homeless individuals living with a disability. Located in the Bronx, the apartments are handicap-adaptable and designed for independent living. With common-spaces including a central garden, this unique site also provides on-site support services, which include substance abuse counseling and case management. Tel: 718-294-1700


• Stratford House
