| Duties: |
Under the supervision of the Program Coordinator, assists clients in obtaining housing and related entitlements/benefits. |

| Responsibilities: |
- Completes initial housing assessments and develops housing service plans.
- Conducts individual and group counseling sessions focused on housing issues.
- Develops and maintains housing resources and assists clients in finding permanent, affordable housing.
- Assists clients in applying for Section 8 and follows up on applications.
- Helps clients apply for various housing programs: HRA-1995; SROs. Follows-up on applications.
- Documents housing progress for all clients.
- Attends interdisciplinary team meetings, case conferences and housing meetings.
- Develops and maintains relationships with outside agencies such as housing providers, realtors, HRA and Section

| Qualifications: |
Bachelors degree and two years relevant experience or A.A. and four years relevant experience. Direct experience in providing client services to homeless, mentally ill and/or chemically addicted population in a residential setting. Bilingual/bi-cultural a plus. |
