| Duties: |
Under the supervision of the Facility Nurse and the Program Physician, the p/t Nurse is responsible for providing medical services for all clients at the program, and specialized medical services for HIV+ clients. |

| Responsibilities: |
- Functions as part of a team of medical professionals to provide medical care to all clients in the facility.
- Orders, dispenses and provides for the security and recording of all prescribed medication.
- Maintains resident medical records and files in accordance with state and agency policies.
- Conducts routine intake physical; participates in developing treatment plans and case conferences.
- Participates in developing and coordinating the medical discharge plans.
- Provides individual counseling and educational services around treatment adherence.
- Assists clients in learning how to manage their medication.
- Develops and maintains linkages with medical centers and community-based agencies who service individuals with HIV/AIDS.
- Maintains professional standing needed to develop quality health care within the agency.
- Compiles the data and prepares reports for the AIDS Institute, NYSDOP and Project Return Foundation, Inc.
- Attends all required training and scheduled meetings.
- Performs other duties as required by the Facility Nurse and the Program Physician.

| Qualifications: |
A NY State license as a Registered Nurse or LPN; 2 years experience providing HIV/AIDS services; experience working with ex-offenders or substance abusers. |
