Program
Shelter Plus Care supports 80+ apartment units throughout New York
City. Case Management services are provided to
all program tenants.
Supportive Housing Special
Needs Program
A permanently subsidized supportive housing program for families
in which one or more of the individuals has an
“Axis I” mental health diagnosis. Participants
receive case management services to assist them with the transition
to independent living and to connect them to other treatment
and entitlement services. The program operates 11
scattered site apartments in the Bronx.
Community Transition Program
Community Transition is a transition-housing program for women in
recovery and their families. The program provides a housing subsidy
for clients for a two-year period. Participants receive general
case management services along with discharge planning into permanent
housing. It includes 8 scattered site apartments in the Bronx
and Manhattan.
