The Community Follow-up Program (CFP), part of Palladia's HIV Services Unit, provides intensive case management services to Medicaid eligible women, men and children who are HIV/AIDS infected or at risk of infection. CFP assists clients in achieving stability and independence while securing quality medical care and support services. 
Program goals are:

  • To provide access to services which increase independence and self-sufficiency.
  • To prevent or delay institutionalization.
  • To increase universal access to HIV information, counseling, testing and services.
  • To monitor patients to ensure access to medical and social services and to promote early medical intervention.

CFP is part of a statewide initiative of the AIDS Institute, New York State Department of Health, under the comprehensive Medicaid case management authority of section 9508 of the Consolidated Omnibus Reconciliation Act of 1985 (C.O.B.R.A.).

Admissions
You may come by appointment or referral. We also accept walk-ins.
• Must be HIV/AIDS or at risk.
• Must provide written verification of HIV/AIDS status.
• Medicaid eligible.
• All ages accepted.
• Minors (under 18 years of age) must have a guardian.
