| Esperanza is a community residence
program for 80 single men providing short term case management and substance abuse services.
Length of treatment is subject to the needs of each individual,
but averages 6 to 9 months. Esperanza offers an array of services
that promote a drug-free lifestyle, a positive sense of self-worth,
and independent living skills. These services include: vocational
and education liaison and referrals, substance abuse counseling,
and assistance in finding affordable permanent housing.
Our interdisciplinary team of professionals is among the finest
in the field. The quality of care we provide includes seasoned managerial,
supervisory, and direct service staff, as well as licensed specialists
in: health care, mental health, housing, vocational education and
substance abuse. Individual treatment plans are designed to address
the individual’s physical and mental health, addiction, legal
and family issues, housing options, educational, employment and
personal needs.
Esperanza is licensed by New York State OASAS.
Services
Assist individuals in locating permanent, affordable housing.
Group and individual counseling services designed to enhance each
person’s mental health while they are coping with the stresses
of recovery, maintaining sobriety and seeking to obtain affordable,
safe and drug-free housing.
Assist individuals in identifying and participating in training
programs that serve to increase their job related skills and employment
opportunities.
Support individuals in improving skills in the following areas:
budget management, shopping, socialization, sobriety maintenance,
family relationships, negotiation and employment maintenance.
Group and individual substance abuse counseling to assist clients in maintaining sobriety. Random urine toxicology testing
is also provided.
Admissions Criteria
Applicant must have a chemical abuse problem requiring chemical dependence services.
Applicant must be homeless at the time of admission.
Applicant must have a minimum of documented thirty days clean and sober.
Applicant will be able to actively participate in the out-patient services for the time they are assigned. Specifically, they will be able to attend individual and group therapy sessions; participate in vocational and /or educational sessions as needed.
Applicant with co-occurring psychiatric and substance abuse disorder (MICA) are welcome, provided their mental health is stabilized.
The individual appears not to be in need of acute hospital care, acute psychiatric care or other intensive services, which cannot be provided in conjunction with community residence or would prevent him from participating in a chemical dependence service.
The individual appears to be free of serious communicable disease that can be transmitted through ordinary contact.
The individual must have demonstrated an ability to participate in or comply with treatment outside of a twenty-four hour setting as indicated by one or more of the following:
- Recent successful attempts at abstinence
- A history of prior treatment episodes, including a demonstrated ability to participate in Outpatient Treatment.
- Substantial functional skills evidencing no need for extensive habilitation or rehabilitation in order to achieve recovery in an independent setting.
